Registration Forms
The Registration Forms page provides a centralized directory of electronic and printable documents essential for managing student records and academic progress. It includes specialized forms for updating personal information, changing majors, requesting graduation, and applying for tuition rate adjustments such as the Maryland Dream Act or in-county business agreements.

Apply to FCC
If you plan to take a credit class you'll need to apply to the college. Once you have been admitted, you may need to take assessment tests depending on your circumstances before meeting with an advisor or enrolling in credit classes.
Please fill out the Academic Suspension Re-Entry Agreement for re-entry after suspension.
Change of Major
Use this form to change your major or catalog year for degree requirements. In order
for your Change of Major to be processed, you must submit this form and meet with
an advisor to review degree requirements and discuss your academic and career goals.

Request for Retroactive Withdrawal Form
Use this form if you would like to request a late withdraw, change to audit on your
transcript or withdraw after the normal refund period.
Verification of Transferability Form
Use this form if you are a current or former FCC student and wish to enroll in a course(s)
at another institution for transfer back to FCC as part of your degree program.
Visiting Students Form
Use this form if you are a student at another college or university, and wish to take
classes at Frederick Community College to transfer back to your home school.
Note: Visiting Students must apply to the college before completing the Visiting Students form.
Graduation Application
Use this form to apply for graduation if you anticipate completing the requirements
for an associate's degree or certificate. All students need to submit a graduation
application to be awarded a credential. Students need to use the graduation application
form versus applying online through PeopleSoft if applying for multiple credentials
or a degree that is not their current major. The deadlines for submitting the application
are published in the academic schedules. Once your application is received, an evaluation
will be completed and a graduation status will be emailed to your myFCC student email
account. (Return form by email, fax, or mail).
Diploma Reprint Request
Diploma Reprint Requests (Use this form to request a reprint of a FCC diploma and/or
to request a Legacy CeDiploma- certified digital diploma) Registration & Records is
pleased to announce a new, convenient diploma reprint request service through a partnership
with Paradigm’s CeCredential Trust. The new service allows graduates to request a
reprint of their FCC credential(s). Through a simple, secured online system, graduates
complete the online form and make payment. Graduates pay a flat fee of $25.00 for
the reprint with shipping included for domestic mailings. International and expedited
domestic shipping is available for an additional cost. Students whose associate and/or
certificate credentials that were posted before August 1, 2022 can order a legacy
digital diploma CeDiploma or CeCertificate. The cost to order a legacy digital diploma
is $50. Select Legacy CeDiploma from the drop down menu. To request a printed credential

Digital Diplomas for Associate and Certificate Degrees
All graduates receiving an associate and/or certificate diploma will receive a printed
diploma and will be able to retrieve a free certified electronic credential known
as CeDiploma or CeCertificate.
Students whose associate and/or certificate credential that was posted on or after August 30, 2022, will receive an email with a link to retrieve their free digital credential when
it is available from our trusted partner, CeCredentialTrust. Availability will be
when the credential has been printed and mailed following the published Graduation
Dates and Deadlines. After the diploma has been printed, graduates will receive an email and information on
how to download their digital credential from [email protected] with a subject line that will read: Subject: Diploma Information: Download your FCC Digital Diploma Now.
Please continue to monitor your email account. To request a new download link, please
use the CeCredential Trust site to resend the link.

Change of Address
Remember, without a correct address on file, you will not be able to receive important
notices, including your bill.
Change of Name
Use this form if your name has changed, and provide the Records and Registration Center
with official documentation. Please submit completed form to the Records and Registration
Center, Building J (fax/mail accepted)
Consent to Release Student Educational Records
Complete the appropriate sections of this form to release the requested information
to a third party. This will authorize release of information contained in the student's
record. This does not give authorization to act on behalf of the student. Form must be submitted by student; student must attach a copy of a photo ID at time
of submission.
